Fillable Form Instructions
Our fillable forms must be opened in Adobe. If you are viewing a “Please Wait” error message, try downloading the form and opening it in Adobe Acrobat or Adobe Reader. You may also try changing your browser’s default settings to open PDF files in Adobe Acrobat or Adobe Reader and not within the browser. View complete instructions on fillable forms below.
Download in Windows computer
- While holding down the “alt” key, click to open the fillable form on the website. Your computer should download the form into your downloads folder.
- Open Adobe Acrobat or Adobe Reader, and open the downloaded form.
To view PDFs with Chrome
- Open Chrome and select the three vertical dots in the top right corner near the address bar.
- Click on Settings
- Under the Privacy and security, click on “Site Settings” then click “Additional content settings.”
- Find “PDF documents” and click on the arrow to expand the menu.
- Turn on the “Download PDF files instead of automatically opening them in Chrome.” The button will be solid blue.
- Close the settings tab, and you are now able to view fully functional PDF files.
To view PDFs in Firefox on Windows
- Choose Tools > Add-ons.
- In the Add-ons Manager window, click the Plugins tab, then select Adobe Acrobat or Adobe Reader.
- Choose an appropriate option in the drop-down list next to the name of the plug-in.
- “Always Activate” sets the plug-in to open PDFs in the browser.
- “Ask to Activate” prompts you to turn on the plug-in while opening PDFs in the browser.
- “Never Activate” turns off the plug-in so it dos not open PDFs in the browser.
- Select the Acrobat or Reader plugin in the Add-ons Manager.
Download to your iPhone or iPad
- Open the Safari Browser on your iPhone or iPad and click on the PDF you wish to download and this will open the PDF file in Safari. If you get a “Please Wait” error message, locate the Share icon located at the bottom left corner of your screen. From the Share Menu that appears, tap Copy to Books.
- Open Adobe Acrobat and on the bottom menu bar, select Files.
- Locate the file on your iPhone or iPad and select it.
Download in Apple computer
- While holding down the “option” key in the Safari web browser, click to open the fillable form on the website. Your computer should download the form into your downloads folder.
- Open Adobe Acrobat or Adobe Reader, and open the downloaded form.
Download to your Android device.
- Click on the PDF link in your web browser. If you get a “Please Wait” error page, find the three vertical dots in the upper right corner next to the address bar.
- Click the three vertical dots, and click Download.
- Open Adobe Acrobat and on the bottom menu bar, select Files.
- Locate the file on your Android device and select it.
To view PDFs with Apple Safari, you can do one of the following:
- Set Safari preferences to use the Adobe Reader plug-in.
- Disable the Adobe PDFViewer plug-in to use the default Safari PDF viewer.
- Set Safari preferences to use the Adobe Reader plug-in to view PDFs.
- Launch Safari, and then choose Preferences.
- In the Preferences window, choose Security, and then click the Website Settings button for Internet plug-ins.
- Now select Adobe Reader in the list of plug-ins
- Under the option When visiting other Websites, in the drop-down list, choose Allow Always and then click Done. NOTE: This will set the browser to use the Adobe Reader plug-in to view PDFs. Disable AdobePDFViewer plug-in to use the default Safari PDF viewer.
To view PDFs in Firefox on Mac OS
- Select Firefox.
- Choose Preferences > Applications.
- Select a relevant content type from the Content type column.
- Select “Save file” or associate the content type with the application to open the PDF. For example, to use the Acrobat plug-in within the browser, choose “Use Adobe Acrobat NPAPI Plug-in.”
To view PDFs in Internet Explorer 8 and later, including Microsoft Edge.
- Open Internet Explorer and choose Tools > Manage Add-ons.
- Under Add-on types, select Toolbars and Extensions.
- In the Show menu, select All Add-ons.
- Select All Add-ons from the Show menu in the Manage Add-ons dialob box.
- In the list of add-ons, select Adobe PDF Reader.
NOTE:
- If you do not see the Adobe PDF Reader add-on, try the other options on the menu. For example, on some systems, the add-on appears when you select Run Without Permission.
- Click the Enable or Disable button (it toggles depending on the status of the selected add-on):
- Enable sets the Adobe PDF Reader add-on to open PDFs in the browser. Disable turns off the add-on so it does not open PDFs in the browser.
- Select Adobe PDF Reader and click the Enable/Disable button.
Microsoft Edge
Microsoft Windows 10 will ship with two browsers: Internet Explorer 11 and the new Edge browser.
The Edge browser will be the default browser, and Internet Explorer 11 will be available to support legacy workflows. The new Edge browser will not have any support for ActiveX plug-ins. Therefore, the Acrobat/Reader plug-in won’t work with Edge. Use Internet Explorer 11 to open PDFs. To enable the Acrobat/Reader plug-in in Internet Explorer, see the steps in the previous section.
