If you meet any of the following criteria, please fill out the Report COVID-19 Exposure form. You will be contacted with instructions.

Ill with COVID-19 symptoms
Have had FIRST HAND exposure to someone who tested positive for COVID-19
Have tested for COVID-19 for ANY REASON and awaiting results or have results
Have tested positive for COVID-19
